Case Management Program

Case Managers at OCMHS help clients increase independence and quality of life by providing the support and motivation needed to deal with mental health issues.

Case Management clients complete a comprehensive assessment and then work with their case manager to identify specific goals. Goals are based on issues that are important to the client, and provide a step by step plan which will reduce barriers that have gotten in the way before. Case Managers then support clients in doing the work to achieve those goals.

Common Goal Areas:

  • Vocational

  • Housing

  • Medical

  • Financial

  • Legal

  • Social

  • Transportation

If you have a diagnosed mental health problem, or feel you may have one and have not yet been diagnosed, submit a referral to one of our offices in either Rumford or Norway, and we may be able to help. To make a referral call 207-364-3549. 

Grant funds may be available if you do not currently have MaineCare.
